Registration System Unavailable Until July 1

The Quest Diagnostics® registration website is currently unavailable as we get ready for the annual Wellness Screening registration period. You may schedule a Know Your Numbers Wellness Screening starting on Monday, July 1, 2025.

Newly Enrolled or Return to Benefits: If you already registered for a wellness screening at a Quest Diagnostics Patient Service Center, you may still visit the Patient Service Center and complete your wellness screening as a walk-in through June 26, 2025. If you do not visit the Patient Service Center on or before June 26, 2025, you will have to wait until July 1, 2025 to register/re-register for an appointment.

If you visited your physician, you may fax or upload a completed Physician Results Form to Quest Diagnostics within 45 days of the date of the letter you received from the PEBTF. Your form will be processed after July 1, 2025 and you will earn the waiver retroactive to the first pay period when you enrolled in PEBTF benefits.
