For Northeastern PA counties, the Custom HMO Northeast is offered by Geisinger for the remainder of 2025. Beginning January 1, 2026, Aetna will offer the statewide Custom HMO. Geisinger will no longer be offered. Please refer to the Plan Choices by Region map to see the counties where the Custom HMO Northeast (Geisinger) is offered for 2025.
