Summary Plan Description (SPD) Request

The Summary Plan Description (SPD) summarizes the main terms of the benefits provided to Members and their eligible Dependents under the Pennsylvania Employees Benefit Trust Fund Plan. This SPD replaces all previous Summary Plan Descriptions for the Plan.

The SPD has been prepared to help you understand the main features of the health benefit coverage provided by the Pennsylvania Employees Benefit Trust Fund (“PEBTF”).
